a) As well as satisfying the guarantees, the Insurer should supply the Policyholder with the Policy or, as the case may be, either the provisional cover or other applicable document as stipulated in article 5 of the Insurance Contract Act, as well as a copy of the questionnaire and other documents that may have been signed by the Policyholder

b) The Insurer shall provide the Policyholder and / or Insured with the Sanitas Health Plan card(s), stating the emergency services information telephone number
